Hi Ian,

The chips are 16C745 and 16C765.
You can find sample code on Microchip's web site at
     http://www.microchip.com/11110/helper.htm

Point your mouse on  'Developer's toolbox'
Select 'Application Notes, Code Examples & Templates'
Then clic 'Firmware drivers'

You will find the following files to download :
USB Firmware (Assembly Version)    usb124as.zip
USB Firmware (C Version)    usb121c.zip
