Two of us here at Current Designs have written a program to do 
in-circuit programming of PIC18F4550 chips via the parallel port.  We'd 
be happy to share it if anyone would find it useful; of course its a 
use-at-you-own-risk offer!

We've used it to successfully load the bootloader that comes with the 
PICDEM FS USB board (MCHPUSB.hex) onto our own board, so that further 
program development can proceed through the usb port.

The whole program is in a single C file.  I'd like to put it somewhere 
where everyone can get at it easily, along with a pdf describing the 
cable I use and a README.txt file.
