I had similar problem today, when I've forgotten to connect the 220nF
capacitor between VUSB and GND in my 18F4550 board.
Incorrect USB base clock frequency may also cause similar problems.
Also make sure that you don't set the FSEN bit in the UCFG register (as
your device works in low speed mode).
