The silkscreen is generated in exactly the same way as the copper 
layers, from the CAD user's perspective. The component libraries contain 
the required silkscreen shapes for packages, and the part numbers are 
included. Sometimes the default part number position is unfortunate so 
you do something to move it (on Eagle it's called "smash", which unlocks 
the annotations from the component and allows you to move them 
independently).

As to the printing of silkscreen, if you get your PCB produced 
professionally from your Gerber plots, it's just a case of supplying the 
extra files. If you're etching the board yourself you don't bother with 
a silkscreen. But then, you don't bother with solder mask layers either, 
or Gerber files, or drill data, or....
